Overview
As a Care Transitions Coordinator (Clinical Liaison), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.
Responsibilities
Serves as the primary relationship manager between the company and referral sources including physicians, physician practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations or progressional. Supports referral growth by providing awareness and education of company services and program requirements to referral sources, patients, families, caregivers and community partners to help navigate post-acute care transitions with the goal of improving patient outcomes, supporting care transition, and promoting exceptional quality and patient / referral source satisfaction.
Qualifications
- Required to have Bachelor's Degree in, healthcare administration, marketing, or related field or a graduate of an approved school of nursing, therapy or social work.
- Highly preferred graduate of an approved school of nursing, therapy or social work.
- Required minimum two years of experience in healthcare business development, community relations, marketing, or sales.
- Required demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.
- Required to have and maintain a valid state driver license in the current state of residence.
- Required to have and maintain automobile liability insurance, as required by law.
- Required strong interpersonal, communication and presentation skills.
- If Clinical:
- Required to have and maintain clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
- Highly preferred experience in home health, hospice, post-acute care, or related healthcare services.
- Preferred previous experience using electronic referral systems and Homecare Homebase (HCHB).
- Preferred established relationships with physicians, hospitals or senior living communities and other referral sources.
- Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
- Oral communication, written communication, fluency in English, active listening.
- Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
- Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
- Enhabit does not waive this essential function by permitting duties to be performed remotely on occasion to accommodate temporary personal needs, such as appointments, or to operate in crises beyond its control, such as during a natural disaster or pandemic
