Responsibilities

Responsible for the review and approval of all patient information submitted by the Licensed Professional (LP) during a start of care, re-certification, resumption of care, or evaluation visit. Partners with branch leadership to provide ongoing education and training to all branch LPs to ensure staff understand documentation
requirements in an effort to meet regulatory standards and to keep staff abreast of changes. When necessary, meet one-on-one with individual LPs to provide in-depth training. Promotes quality patient care; ensures stable relationships with referral sources, patients, caregivers and employees

Qualifications

  • Required graduate of an approved school of professional nursing or physical therapy.
  • Required to have and maintain a valid clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
    • Experience as a licensed professional may be deferred with a review and approval from the vice president operations or regional president
  • Required to have minimum of one year of experience.
  • Required previous experience in a Medicare home health or hospice setting.
  • Required to have and maintain CPR certification, as required by position.
  • Required to have and maintain a valid state driver license in the current state of residence
  • Required to have and maintain automobile liability insurance, as required by law
  • Preferred previous experience with packet review / quality improvement.
  • Preferred previous experience with coding.
  • Required to have basic demonstrated technology skills, including operation of a mobile device.
  • Preferred previous experience using Homecare Homebase (HCHB).
  • Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
  • Oral communication, written communication, fluency in English, active listening.
  • Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
  • Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
    • Enhabit does not waive this essential function by permitting duties to be performed remotely on occasion to accommodate temporary personal needs, such as appointments, or to operate in crises beyond its control, such as during a natural disaster or pandemic.
