Responsibilities

Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures. Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit. Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees. Ensure scheduling of patients to field clinicians and communication with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of
care.

Qualifications

  • Required graduate of an approved school of professional nursing.
  • Required to have and maintain a valid clinical license, per state requirements, or in accordance with the board of nursing rules for a compact nurse license.
    • Experience as a licensed professional may be deferred with a review and approval from the vice president operations or
      regional president
  • Required to have previous experience practicing under the professional license for this position.
  • Required to have minimum of two years of nursing experience within the immediate previous 36 months.
  • Required to have minimum of one year of experience with home health.
  • Required to have demonstrated knowledge and understanding of federal, state and local laws and regulatory guidelines that govern the operation of a home care office.
  • Required to have and maintain CPR certification, as required by position.
  • Required to have and maintain a valid state driver license in the current state of residence
  • Required to have and maintain automobile liability insurance, as required by law
  • Required to have basic demonstrated technology skills, including operation of a mobile device.
  • Preferred previous experience in a management position.
  • Preferred previous experience using Homecare Homebase (HCHB).
  • Preferred bilingual (English/Spanish) with the ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
  • Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
  • Oral communication, written communication, fluency in English, active listening.
  • Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
  • Other than for designated full-time remote positions, in-office attendance is essential in order to achieve
    Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
    • Enhabit does not waive this essential function by permitting duties to be performed remotely on occasion to accommodate temporary personal needs, such as appointments, or to operate in crises beyond its control, such as during a natural disaster or pandemic
